PACE is a program designed exclusively for adults over 55 who live in our service zip codes. (Click Here to See Our Service Area)

Participants must require additional assistance with activities of daily living (ADL) or need supervision for physical, cognitive, or behavioral health concerns.

Participants need to be eligible for nursing home services by the State of Colorado.

Do you have a patient or client you are worried about when they leave your office?

PACE’s built-in all-inclusive care coordinates social care, healthy meals, medication management, and transportation to and from our Day Centers. We also provide home care to help our participants continue to live at home, and their medical appointments are all coordinated along with medical transportation and a medical liaison.

If you have a older patient who you are worried about, reach out to our Enrollment Team with questions or submit an on-line referral.
