Get to know Rocky Mountain PACE

PACE means Programs of All Inclusive Care for the Elderly.

Rocky Mountain PACE is approved to participate in Medicare and Medicaid programs.

PACE supports medical, social, emotional and nutritional needs.

PACE care teams include 11 professional disciplines. These teams meet in one room to review individual care plans and changes in health, social, and emotional well-being.

PACE began 45 years ago in San Francisco, with On Lok Lifeways.

Our goal is to help you remain in your home for as long as you can be safely served.

All of our programs have worked toward this same goal since 1976.

RM PACE provides and coordinates all your medical care.

Your personal care through your IDT will include some or all of the services listed below.

If you have a medical need not on this list, your IDT will work with you to meet your needs.

  • Physicians: Primary care physician, specialists, and nursing care.
  • Medications: Prescription medications, delivery to your home.
  • Laboratory work: Outpatient labs, blood tests and X-rays.
  • Professional services: Dentistry, optometry, audiology, podiatry and psychiatry.
  • Medical Equipment: Wheelchairs, walkers and other durable medical equipment.
  • Rehabilitation: Physical, occupational and speech therapy.
  • Hospitalization: Emergency care, hospitalization, transportation.

RM PACE provides socialization, nutrition, transportation and other social services:

  • Transportation: To and from your neighborhood RM PACE day center, RM PACE medical clinics, and outside medical appointments.
  • Recreation: Art, drama, music, events, exercise, field trips.
  • Home care: Help with personal care and light housekeeping.
  • Nutrition: Hot meals at RM PACE  day centers and nutritional counseling.
  • Spiritual Care: Counseling, family support, help with benefits, and a chaplain.
  • Logistics: We manage it all — the paperwork, the prescription refills, getting people to and from medical appointments, and communication among the entire care team.

When you enroll with RM PACE, all your healthcare will come through our doctors, nurses, and network of specialists.

If you receive services that are not pre-authorized, or are outside of our network, you will be personally responsible for paying.

At RM PACE, all your care providers talk to each other. Each Participant is assigned an Interdisciplinary Team, or IDT for short.

Your doctor and your entire care team have regular meetings. They meet face-to-face, in one room, around a table.

Your care team includes your:

  • Physical Therapist

In-Home Care

At RM PACE, in-home care is personalized for your medical needs, and for what you need to live safely at home. We help everyone differently.

Maybe you need help bathing or getting dressed.

Perhaps you need help getting dressed and ready for our transportation team to take you to a doctor’s appointment.

You might need your medications organized, or your blood pressure monitored.

You might need your laundry done or a light meal prepared.

These are some of the things our in-home care aides do for our participants.

Sometimes living safely at home means you need extra help.

In-home care is given to people who need it, for medical reasons. When you join RM PACE, your medical needs will be assessed and a personal care plan created with you.

Not everyone enrolled at RM PACE receives in-home care.

We’ll know after we create your personalized care plan.

Our professional care team will work with you to create a personalized care plan, and that plan will detail the in-home care you may receive.

Your in-home care aide is an important part of your care team, someone who understands your needs.

Your home care aide is a certified nursing assistant. When needed, a registered nurse or licensed vocational nurse may also visit you.

Care aides are trained professionals. Your home care aide is an employee of RM PACE, committed to your care.
