Starting the day with Rocky Mountain PACE
Your day at Rocky Mountain PACE begins with a phone call from your friendly transportation specialist, who will let you know that we're on our way to pick you up in one of our vans or buses.
Our drivers will assist you at your residence and safely transport you to one of our Day Centers or your specialty appointments.
Health & Wellness
At Rocky Mountain PACE, prioritizing your health and wellness is one of our main goals, as we want you to maintain your independence for as long as possible.
Our on-site team of providers are dedicated to ensuring you stay in peak health. From your primary care provider to your physical, occupational, and recreational therapists, we are all here to make your visits both enjoyable and beneficial for your well-being.
Day Center Activities
As soon as you enter our Day Centers, you will be offered a hot or cold drink to help you get settled in. During each visit, you can enjoy a meal prepared by our culinary staff. We offer a variety of activities, including visits from musicians, bingo, and games like bowling and cornhole. There are also movies and visits from therapy animals and even zoo animals.
During your visit, we can schedule a clinic visit, foot care appointment, dental hygienist appointment, or physical therapy session. There is always something happening at Rocky Mountain PACE!
Heading Back Home
After your visit, we will take you home. We provide door-to-door services, ensuring that we only leave once you are safely inside your home. Your relationship with Rocky Mountain PACE goes beyond a single-day visit; we are available 24/7 to assist you with any questions or concerns.
We offer both medical and non-medical in-home care as prescribed by your Interdisciplinary Team. It’s reassuring to know that we are always here for you if you need us.
