From the very beginning, our team is here to educate and walk you and your family through our enrollment process.
Our participants come to our Centers for medical appointments, activities, and to socialize with their friends in our Day Centers.
We are here to answer any questions you may have, whether you are looking to join our program, or if you are already a participant.

Your doctors, nurses, specialists and pharmacists all work from one personalized care plan. Every member of your team understands your needs.
Live Better
Each week, your care team meets to review care plans, changes in medical conditions, social engagement, and emotional well-being.
At home and in your community
It can be difficult keeping your medications organized or getting to your appointments. We work together to keep you supported at home.
Cost is not a Barrier
All eligible individuals age 55 years of age and older residing in our service areas are are welcome to participate and we can assist in determining your qualifications for Medicare and Medicaid and assist in the application process. A private pay option is also available.
We make it easy for professionals and healthcare providers to refer local older adults to Rocky Mountain PACE.
